Common Form 0877 Mistakes—And How You Can Avoid Them!

All businesses applying to register with the Vendor Information Pages (VIP) database must electronically sign the Verification application, commonly referred to as the 0877 form.

Many owners make mistakes in filling out the 0877 form resulting in application errors and a delay in verification.

Here are some common 0877 mistakes and ways to avoid them:

Owner enters wrong Social Security Number. Red arrow point right Be extremely careful and diligent when entering any personal information.  Double check all work.
Owners are not listed in order of percentage amount. Red arrow point right Owners must be listed in highest order of percentage amount, with the majority owner listed first.
Owner submits incorrect ownership percentages.  This generally happens when there is a change in ownership. Red arrow point right Check to make sure all ownership percentages add up to a total of 100%.
Veteran does not enter name as it appears on the DD-214 form. Red arrow point right Enter your name exactly as it appears on the DD-214 form.  DO NOT input nicknames, aliases or married name if it is not on the DD-214.
Multiple owners enter the same email address on the 0877. Red arrow point right Businesses with multiple owners must have separate and unique email addresses for EACH owner.
Veteran enters wrong Veteran status owner type.  (For example, they are a VOSB, but enter SDVOSB.) Red arrow point right Always check to make sure you input the correct Veteran status owner type.
Veteran enters wrong birth date. Red arrow point right Double check to be sure that your birth date is correct and matches what is on the DD-214 form.

Although these are simple mistakes, they can severely impact the timeliness of your verification process.  When you input correct information the first time, the Center for Verification and Evaluation (CVE) can process your application within 60 days.  However, if you do make a mistake on the 0877 form, the CVE’s Contact Center is able to resolve these problems.  Please call the Help Desk at (866) 584-2344 from 8:00am to 8:00pm (Eastern) with any issues or questions.
